Please also remember -

* All divers doing PADI Courses must complete the PADI medical questionnaire. If you need to answer "yes" to any statement, you must obtain your doctor's signature & details on page 2 of the attached form and bring this with you. If you can answer "no" to all statements, you can complete the form on the day.

* Due to the remoteness of the area, all divers must show proof of diving insurance. This could be either a specific diving insurance policy or cover under travel insurance. Blue Guru can arrange diving insurance for just 200Bt per day.

* All fields with red stars are mandatory.

* All diving courses & trips must be pre-booked to ensure availability of instructor, materials & boat.

* Any additional courses or equipment extras booked during your trip must be paid for in Thai Bhat cash. 

* Deposit & balance payments must be paid by bank transfer. Deposit payments are required to secure booking. Balance payments are due 2 months before trip date.

* National Park fees & equipment rental will be payable in Thai Baht cash upon check in.
