Important - please note
* All divers must complete the PADI medical questionnaire. If you need to answer "yes" to any statement, you must obtain your doctor's signature & details on page 2 of the attached form and bring this with you. If you can answer "no" to all statements, you can complete the form on the day. If you do need to answer 'yes' to a medical question but do not obtain doctor's confirmation of fitness before arrival, then we will not be able to take you diving and you will forfeit your payment.
* All certified divers must bring their dive certification card to be presented at Blue Guru Diving on arrival.
* Due to the remoteness of the area, all divers must show proof of diving insurance. This could be either a specific diving insurance policy or cover under travel insurance. Blue Guru can arrange diving insurance for just 200Bt per day.
* All fields with red stars are mandatory.
* All diving courses & trips must be pre-booked to ensure availability of instructor, materials & boat. Surin, Tachai & Richelieu dive trips are conducted from a large dive cruiser. These trips are run subject to minimum numbers.
* All prices are subject to 10% service fee
* Any additional diving, activities or trips booked during your trip must be paid for in Thai Bhat cash. Blue Guru cannot accept credit card payments in person.
* There are no ATMs/banks on Koh Phra Thong, so sufficient cash should be brought to the resort
