* All divers must complete the PADI medical questionnaire. If you need to answer "yes" to any statement, you must obtain your doctor's signature & details on page 2 of the attached form and bring this with you. If you can answer "no" to all statements, you can complete the form on the day.
* Due to the remoteness of the area, all divers must show proof of diving insurance. This could be either a specific diving insurance policy or cover under travel insurance. Blue Guru can arrange diving insurance for just 200Bt per day.
* All fields with red stars are mandatory. It may not be necessary to fill out 'pick up' or 'drop off' places if they have already been arranged as part of a large Blue Guru Diving holiday package.
* All diving courses & trips must be pre-booked to ensure availability of instructor, materials & boat. Surin & Richelieu dive trips are conducted from a large dive cruiser. These trips are run subject to minimum numbers.
* All prices are subject to 10% service fee
* Any additional diving, activities or trips booked during your trip must be paid for in Thai Bhat cash. Blue Guru cannot accept credit card payments in person.
* There are no ATMs/banks on Koh Phra Thong, so sufficient cash should be brought to the resort
